Navigator Role and Goals

The Community-Based Social Service Navigator role will serve as a resource to increase education and outreach to help navigate existing social services and bridge gaps of inequity.  Navigators will meet people where they are and connect them to resources and services.  The Navigator is a trusted, culturally and technologically knowledgeable community partner targeting outreach to our most vulnerable community members.

Step 1: Initial Contact

You can reach out to us through various channels, including flyers, outreach events, referrals, or directly through our website. Our intake process begins the moment you decide to seek assistance.

Step 2: Providing Your Information

To serve you better and tailor our assistance to your needs, we will require some specific information during the intake process. Please be prepared to provide the following details:

  • Basic Information:

    • First and Last Name

    • Email Address

    • Street Address

    • City, Zip Code

  • Personal Background:

    • Ethnicity (please mark all that apply)

    • Military discharge status (if applicable)

    • Primary language spoken

  • Household Information:

    • Number of family members residing with you

    • Current housing situation

    • Concerns about housing stability

  • Employment and Health:

    • Current work situation

    • Main health insurance provider

  • Financial Information:

    • Combined household income over the past year (to assess eligibility for benefits)

  • Specific Needs and Concerns:

    • Access to necessary items and services (food, transportation, utilities)

    • Safety concerns, both physical and emotional

    • Any other services or support needs you have

Step 3: Malamalama — Compassionate Listening

Our trained staff will engage in 'Malamalama' which means to enlighten with knowledge. We listen to your story and struggles with empathy and understanding, aiming to get a holistic view of your situation.

Step 4: Connecting You to Services

Based on the information provided and our compassionate listening session, we will assist you in connecting with the appropriate social services. We will outline which services we can help you access and any additional steps needed.

Step 5: Follow-Up and Support

After the initial connection, we will follow up to ensure that you have been able to access the services and provide additional support if needed.
